1. Print and fill out the Student Nomination Form and submit by January 15, 2009.
   
   
2. Have a teacher or guidance counselor nominate you by writing a letter of recommendation.
   
   
3. Include an autobiographical sketch (500 words or less).
   
   
4. Include a copy of your transcript.
